That error 5 is being sent from the TQsl application. I tried to search to see what may be the cause it wasn’t a lot of help, it indicated in some places that that your certificate may be expired and in another that you need to enter a password for upload in the TQsl app and maybe that wasn’t right.
When you select to upload do you see the TQsl application launch? I know for me it does - behind QLog on my Mac but it waits for me to select the appropriate Lotw location to continue.

Download works ok from LOTW. My tqsl is on C:/Program Files (x86)/TrustedQSL/tqsl.exe

Download works ok from LOTW. My tqsl is on C:/Program Files (x86)/TrustedQSL/tqsl.exe
